Position Type: Full-time (40 hrs per week)

Reports to: Associate Director

The Family & Natural supports program strives to work with youth and their families and/or natural supports in an effort to strengthen or develop positive relationships. Youth work alongside the FNS Coordinator to identify systems and positive persons of support to gain new tools for relationship development, engagement with others and conflict management. Throughout the process the FNS Coordinator will work to achieve goals laid out in client specific case management plans to create or stabilize family and natural support .

Program Goal

The goal is to ensure that by cultivating positive connections between youth and their family and natural supports we can begin to prevent any further youth from becoming homeless and entering into the emergency housing system in the Yellowknife region. FNS Is part of a larger research project, this means that participants and their families will have the opportunity to engage in research interviews during the case management process, as well as afterwards.

Responsibilities

  • Work a flexible 40 hour week to meet the individualized needs of the families served
  • Adhere to agency guiding principles and best practices for client centered and strength based practices, promoting relationships that foster resilience, responsibility, and independence
  • Assist youth and their natural supports through case management in developing and implementing service plans, discharge plans, safety planning, goal setting, problem solving, conflict resolution, and communication skills
  • Provide information, articles, books, and other materials needed to assist natural supports in enhancing skills and knowledge on parenting, adolescent development, and other applicable topics
  • Advocate for youth and their natural supports to promote increased functioning, access to equitable and required resources, and successful transition to adulthood
  • Work collaboratively with other agency partners to provide case management to youth and supports including meetings, case conferences, and training sessions as necessary
  • Develop and maintain professional relationships with external agencies and groups such as community associations, schools, law enforcement and social service agencies in addition to seeking out professional contacts that will benefit programming
  • Act as a liaison between families, social workers, school representatives, doctors, and other professionals involved with the youth and their supports to promote self-sufficiency
  • Ensure that all documentation, critical incident reporting, and assessments are kept current, complete and professional
  • Manage crisis situations effectively using problem solving and de-escalation skills in alignment with agency Policies and Procedure
  • Maintain fiscal responsibility through adherence to the program budget and submit all financial paperwork and reports in a timely manner.

Qualifications

  • Minimum of 19 yrs of age required
  • First Aid/ CPR asset
  • Vulnerable Sector Check (within past 6 months)
  • Class 5 License with at least one year driving experience required
  • Clear Driver’s Abstract required
  • Fluency in English (comprehension, written, spoken) required
  • High School Diploma required
  • Secondary Education, Diploma or Degree, in Youth Work, Social Work or other Human Services field asset
  • Relevant lived experience in Human or Social Services field (will be evaluated on an individual basis)
  • Two to five years experience in a Human Services Field required
  • One year Management or Supervisory level experience required
  • Fluent in the use of Google Doc Suites, graphic arts program (Canva)
  • Experience completing intake paperwork, writing case notes, implementing individualized case planning
